PROVIDENCE IN-HOME CARE SERVICE
Position Summary
PROVIDENCE IN-HOME CARE SERVICE is seeking a compassionate, organized, and detail-oriented Remote Care Manager to oversee client care coordination and support caregivers remotely. The Remote Care Manager serves as the primary point of contact for clients, family members, caregivers, and healthcare professionals to ensure high-quality, personalized in-home care services.
This position requires excellent communication, problem-solving, and case management skills, along with a commitment to delivering exceptional client care and maintaining compliance with company policies and industry standards.
Key Responsibilities
Care Coordination & Client Support
- Conduct client intake assessments and develop individualized care plans.
- Monitor client care services and ensure care plans are implemented effectively.
- Maintain regular communication with clients, families, caregivers, and healthcare providers.
- Address client concerns, service issues, and care-related inquiries promptly.
- Schedule and coordinate caregiver assignments based on client needs.
Caregiver Management
- Support and supervise caregivers remotely.
- Monitor caregiver performance and provide guidance and coaching as needed.
- Ensure caregivers comply with company policies, procedures, and care standards.
- Assist with caregiver onboarding, orientation, and ongoing training.
Documentation & Compliance
- Maintain accurate and confidential client records.
- Document client interactions, care updates, incidents, and service changes.
- Ensure compliance with state regulations, HIPAA requirements, and company policies.
- Prepare reports and maintain quality assurance records.
Communication & Quality Assurance
- Conduct regular wellness checks with clients and caregivers.
- Monitor service quality and client satisfaction.
- Collaborate with healthcare professionals and community resources to support client needs.
- Participate in quality improvement initiatives and case reviews.
Qualifications
- Associate's or Bachelor's degree in Healthcare Administration, Nursing, Social Work, Human Services, or a related field preferred.
- Minimum of 2 years of experience in home care, healthcare coordination, case management, or client services.
- Strong knowledge of home care operations and client care practices.
- Excellent verbal and written communication skills.
- Strong organizational and multitasking abilities.
- Proficiency in Microsoft Office, scheduling software, and electronic health record systems.
- Ability to work independently in a remote environment.
Preferred Skills
- Experience in home health care or senior care services.
- Knowledge of Medicaid, Medicare, and long-term care programs.
- Crisis management and conflict resolution skills.
- Bilingual abilities are a plus.
